sharepoint - CSOM AuthenticationManager doesn't ask for sms-code or authenticator

Question

I have implemented the authentication flow using AuthenticationManager in a Windows Forms app using CSOM. Works very good and supports MFA as it should, except for in one tenant (so far). In that tenant it starts the flow:

1. Opens the window and asks for username.
2. Switch to password window.
3. Blank window ( here it should ask for the smscode sent to the mobile device).

Works in other tenants. Also works outside the Windows Forms App, ie. in a plain browser.

Maybe some kind of policy that applies but I have tried to recreate the error with no luck. Any suggestions?
